Three, each doing one job
The league publishes a Team Waiver as its own registration step, a Rules section, and an Umpire Evaluation form -- three documents, each attached to the process it belongs to rather than filed in a downloads folder.
Separating the team waiver from the player registration is the sort of distinction a league only makes after something has gone wrong once.
Documents belong beside the season they govern. A High Cheese league page collects the waiver as part of registration and keeps the rules attached to the year being played.
