A tie-break chain clarified by the board and never written into the bylaws.

The rule they fixed and forgot to file

The league published a clarification that is more revealing than any rulebook. Section 9 of its bylaws said a tie would be settled by one playoff game. The commissioner spoke to each board member and reported that the section had been clarified two years earlier but never updated in the bylaws — and would be. The actual order, he set out, is percentage points, then head to head, then runs against, then one playoff game, similar to the section covering three-way ties. He also noted the league had never actually had a tie.

Publishing the discrepancy between what the bylaws say and what the board decided, rather than quietly amending it, is unusually honest. It is also exactly the failure a written record prevents.
