The forms page carries the OSBA Constitution, the association's Concussion Designated Persons Policy, Baseball Ontario's Removal From Sport Protocol and Return to Sport Strategy, the 2026 OSBA Game Stat Sheet, and an Incident Report.
That combination says a great deal. A league that has named designated concussion persons and adopted its provincial body's removal and return protocols has decided that men in their thirties and forties get the same head-injury care as minors, and the incident report exists because somebody has to write down what happened.
The game stat sheet is the other end of the same seriousness -- it is the paper that feeds the weekly pitcher-of-record reports and the season packages.
A stat sheet filled in on a phone at the ground, feeding the standings and the career leaders directly, is what a High Cheese league page replaces it with, and the constitution and the protocols sit at the same address rather than on a forms page nobody opens twice.
