Do not write to anything at the old address
Any email or form still reachable at the old domain belongs to whoever took it, not to a baseball league, so nothing on it should be filled in.
We hold no officer, phone number or address for this league. If somebody who ran it wants a working contact back on the record, a claimed page on High Cheese is a contact the league controls at an address that does not expire out from under it.
